Appointment, Cancellation, and Attendance Policy

At Crescent Urology Institute, we value your time and strive to provide timely, high-quality care to all our patients. To help us maintain an efficient schedule, we have established the following appointment policy.

Appointment Rescheduling

  • Patients who reschedule an appointment with at least 24 hours’ notice will not be charged a fee.

  • Patients who choose to reschedule an appointment with less than 24 hours’ notice will be charged a $75 rescheduling fee.

  • If an appointment is rescheduled, the patient will be scheduled for the next available appointment, which may not be immediate.

  • Appointment availability is based on provider schedules and clinical demand.

Appointment Cancellations

  • Patients who cancel an appointment with at least 24 hours’ notice will not be charged a fee.

  • Patients who cancel an appointment with less than 24 hours’ notice will be charged a $75 cancellation fee.

No-Show Policy

  • A no-show is defined as failing to arrive for a scheduled appointment without prior notice.

  • Patients who do not show up for their appointment will be charged a $75 no-show fee.

  • After three (3) no-show appointments, the patient may be dismissed from the practice at the discretion of Crescent Urology Institute.

Late Arrival Policy

  • Patients are expected to arrive on time for their scheduled appointment.

  • A 15-minute grace period is allowed.

  • Patients arriving more than 15 minutes late will be charged a $25 late fee.

  • Late arrivals may need to be rescheduled if the provider is unable to accommodate them. If rescheduled with less than 24 hours’ notice, a $75 rescheduling fee may apply.

Payment Responsibility

  • All applicable fees are the patient’s responsibility and are not typically covered by insurance.

  • Fees may be charged to the card on file or billed directly to the patient.
