Appointment, Cancellation, and Attendance Policy
At Crescent Urology Institute, we value your time and strive to provide timely, high-quality care to all our patients. To help us maintain an efficient schedule, we have established the following appointment policy.
Appointment Rescheduling
Patients who reschedule an appointment with at least 24 hours’ notice will not be charged a fee.
Patients who choose to reschedule an appointment with less than 24 hours’ notice will be charged a $75 rescheduling fee.
If an appointment is rescheduled, the patient will be scheduled for the next available appointment, which may not be immediate.
Appointment availability is based on provider schedules and clinical demand.
Appointment Cancellations
Patients who cancel an appointment with at least 24 hours’ notice will not be charged a fee.
Patients who cancel an appointment with less than 24 hours’ notice will be charged a $75 cancellation fee.
No-Show Policy
A no-show is defined as failing to arrive for a scheduled appointment without prior notice.
Patients who do not show up for their appointment will be charged a $75 no-show fee.
After three (3) no-show appointments, the patient may be dismissed from the practice at the discretion of Crescent Urology Institute.
Late Arrival Policy
Patients are expected to arrive on time for their scheduled appointment.
A 15-minute grace period is allowed.
Patients arriving more than 15 minutes late will be charged a $25 late fee.
Late arrivals may need to be rescheduled if the provider is unable to accommodate them. If rescheduled with less than 24 hours’ notice, a $75 rescheduling fee may apply.
Payment Responsibility
All applicable fees are the patient’s responsibility and are not typically covered by insurance.
Fees may be charged to the card on file or billed directly to the patient.
